New Patients: Please arrive 30 minutes before the appointment to get checked in. If you complete the office forms prior to arriving, this will expedite your check in process.
Existing Patients: Please arrive 15 minutes before your appointment to verify information.
Missed appointments will be subject to a fee of $50 for Provider’s appointments and $25 for labs appointments if not cancelled at least 24 hours in advance.
